Roberts Ink tattoos clients age 16 and older. Any client who is 16 or 17 must have this form completed and signed by a parent or legal guardian, and the parent/guardian must be present at the appointment with a valid photo ID.

I understand a tattoo is permanent and carries risks including infection, allergic reaction, and scarring.
